    Background:

    • Bell's palsy involves facial nerve weakness, but underlying auditory pathway integrity is not fully understood.
    • Standard audiometric evaluations may not detect subtle neural pathway dysfunction.

    Observation:

    • Auditory brain-stem evoked potentials (ABEP) were recorded in 24 Bell's palsy patients within one week of symptom onset.
    • Audiometry showed no abnormalities, but 25% of patients exhibited significant ABEP deviations.

    Findings:

    • ABEP abnormalities indicated prolonged brain-stem conduction time, sometimes only detectable at higher stimulus rates.
    • ABEP results were not correlated with paralysis severity, side, acoustic reflex, or dysacusis.
    • ABEP did not predict facial muscle function recovery.

    Implications:

    • ABEP can detect subclinical auditory pathway dysfunction in Bell's palsy patients missed by standard hearing tests.
    • These findings suggest Bell's palsy might be a symptom of a more widespread neurological condition.
