    A novel follow-up system for hydatidiform mole patients in Chiba prefecture helped avoid over-treatment in 36.3% of cases. This system accurately identified patients needing treatment for trophoblastic sequelae, including choriocarcinoma and invasive mole.

    Context:

    • Since 1974, Chiba University School of Medicine has managed trophoblastic disease patients.
    • An original follow-up system for postmolar patients was implemented.
    • Between 1974-1981, 1544 hydatidiform mole cases were registered, with an incidence of 1:377 deliveries.

    Purpose:

    • To evaluate an original follow-up system for postmolar patients.
    • To assess the incidence of trophoblastic sequelae and spontaneous remission.
    • To determine the effectiveness of the follow-up system in preventing over-treatment.

    Summary:

    • Patients with hCG titers >10,000 IU/l (5 weeks), >1,000 IU/l (8 weeks), or >100 IU/l (12 weeks) post-evacuation were monitored for trophoblastic sequelae.
    • 10.0% of cases (155/1544) were treated as sequelae, including choriocarcinoma (0.6%), invasive mole (3.5%), and undetermined (6.1%).
    • Spontaneous remission was observed in 31.2% of cases with hCG 1,000-10,000 IU/l at 5 weeks and 41.9% with hCG 100-1,000 IU/l at 8 weeks.

    Impact:

    • The implemented follow-up system successfully prevented over-treatment in 36.3% (66/1544) of registered patients.
    • Accurate identification of trophoblastic sequelae and spontaneous remission cases.
    • Improved management protocols for hydatidiform mole and subsequent trophoblastic disease.
