    Summary

    Midface advancement surgery (LeFort III) showed stability in most patients initially, but long-term follow-up revealed some relapse and positional changes. Further procedures were sometimes needed for occlusion and orbital issues.

    Background:

    • Midface advancement (LeFort III osteotomy) is a surgical technique used to correct midfacial hypoplasia.
    • Long-term stability and outcomes of this procedure require further investigation.

    Purpose of the Study:

    • To evaluate the long-term cephalometric stability of midface position following LeFort III advancement.
    • To assess the incidence of midface relapse and need for secondary interventions.

    Main Methods:

    • Retrospective study of 19 patients who underwent LeFort III advancement between 1972 and 1980.
    • Cephalometric data analyzed immediately postoperatively and over a mean follow-up of 5.8 years (range 2-11 years).

    Conclusions:

    • LeFort III advancement demonstrates variable long-term stability, with a tendency for downward and/or forward positional changes.
    • Secondary procedures may be necessary to address occlusal issues and residual skeletal discrepancies.
    • Mandibular growth is a significant factor influencing the final facial profile post-midface advancement.
