    • The condition was characterized by benign submucosal cysts lined by columnar epithelium.
    • This benign finding differentiated it from malignant rectal cancer.

    • Colitis cystica profunda is a rare but important differential diagnosis in ulcerative colitis patients presenting with rectal obstruction.
    • Accurate histological diagnosis is essential to avoid unnecessary radical surgery.
    • A conservative surgical approach is recommended for benign colitis cystica profunda.
