    Summary

Tuberculosis can mimic cancer, presenting with subtle symptoms and uncommon signs. Early diagnosis is crucial, as this elusive disease often requires specific testing beyond initial evaluations for neoplasms.

    Area of Science:

    • Pulmonology
    • Infectious Diseases
    • Oncology

    Background:

    • Neoplasms are often suspected in patients with persistent symptoms.
    • Tuberculosis (TB) can present atypically, mimicking other serious conditions.
    • Delayed diagnosis of TB can lead to adverse outcomes.

    Purpose of the Study:

    • To analyze the clinical presentation of tuberculosis (TB) in patients initially suspected of having neoplasms.
    • To highlight diagnostic challenges and delays in TB cases presenting with malignancy-like symptoms.

    Main Methods:

    • Retrospective chart review of 26 patients.
    • Analysis of patient demographics, symptoms, diagnostic procedures, and final diagnoses.
    • Comparison of clinical findings with presumptive diagnoses of neoplasms.

    Main Results:

    • 81% of patients were US-born with limited TB exposure history.
    • Subtle symptoms (2.8 months duration) were common; classic TB signs were rare.
    • Pulmonary TB (14 patients) and lymphadenitis (9 patients) were most frequent; 62% had chest abnormalities.

    Conclusions:

    • Tuberculosis can present insidiously, often misdiagnosed as malignancy.
    • Lack of TB-specific testing (e.g., skin tests) and subtle presentations contribute to diagnostic delays.
    • Increased awareness and appropriate testing are vital for diagnosing TB, especially when malignancy is suspected.
