    • Four patients (three male, one female) presented with postaxial polydactyly and demonstrated subnormal growth hormone responses.

    • One patient exhibited a severe malformation syndrome with a central nervous system (CNS) hamartomatous tumor and died early, without a family history of polydactyly.

    Findings:

    • A significant association was observed between postaxial polydactyly and hypopituitarism in the studied cohort.
    • The presence of CNS hamartomatous tumors may be linked to this specific phenotype.
    • Genetic factors likely play a role, though sporadic cases with severe malformations also occur.

    Implications:

    • The co-occurrence of postaxial polydactyly and hypopituitarism should be considered during the diagnostic workup of children with growth hormone deficiency.
    • This finding may refine diagnostic criteria and genetic counseling for patients with polydactyly and endocrine abnormalities.
