    Summary

This study modified an electronystagmograph to objectively estimate visual acuity by inhibiting optokinetic nystagmus. The method successfully verified suspected visual acuity malingering or exaggeration in most patients.

    Area of Science:

    • Ophthalmology
    • Neuroscience
    • Medical Technology

    Context:

    • Assessing visual acuity is crucial for diagnosing and managing various ocular and neurological conditions.
    • Malingering or exaggeration of visual deficits presents a diagnostic challenge in clinical practice.
    • Existing methods for visual acuity testing can be subjective and prone to manipulation.

    Purpose:

    • To develop and validate an objective method for estimating visual acuity.
    • To utilize a modified electronystagmograph and Ohm's method for visual acuity assessment.
    • To detect suspected simulation or aggravation of visual acuity deficits.

    Summary:

    • A modified Jung electronystagmograph was employed to objectively measure visual acuity.
    • The technique involved inhibiting optokinetic nystagmus using a variable light spot projected on a screen (Ohm's method).
    • The modified method verified suspected malingering or exaggeration of visual acuity in 95 out of 98 patients (161 eyes).

    Impact:

    • Provides a reliable objective tool for visual acuity assessment, aiding in accurate diagnosis.
    • Offers a method to differentiate genuine visual impairments from feigned conditions.
    • Enhances the diagnostic capabilities in ophthalmology and neurology, particularly in cases of suspected malingering.
