    Summary

This study reports a rare case of chronic urticaria with angioedema and renal involvement, characterized by specific inflammatory markers and low complement levels. Treatment with corticosteroids effectively resolved the patient's symptoms and proteinuria.

    Area of Science:

    • Immunology
    • Nephrology
    • Dermatology

    Background:

    • Atypical chronic urticaria can present with systemic manifestations beyond skin symptoms.
    • Hypocomplementemia, characterized by low complement levels, can indicate underlying autoimmune or inflammatory processes.

    Observation:

    • A 58-year-old woman presented with chronic urticaria, arthralgias, abdominal pain, and angioneurotic edema.
    • She exhibited signs of inflammation including leukoneutropenia, antileukocyte antibodies, and low complement levels (CH50, C1q, C4).
    • Skin biopsy revealed angiitis with a positive lupus band test, and renal biopsy showed mesangial deposits and inflammatory infiltrates.

    Findings:

    • Despite negative serologic tests for Systemic Lupus Erythematosus (SLE), the patient's presentation and biopsy results suggested an autoimmune-mediated vasculitis.

    Related Experiment Videos

  • The observed hypocomplementemia was not due to a functional C1 esterase deficiency.
  • Renal biopsy findings indicated immune complex deposition and inflammation.
  • Implications:

    • This case highlights McDuffie's angiitis as a differential diagnosis for chronic urticaria with systemic involvement.
    • It underscores the importance of comprehensive investigation, including complement studies and biopsies, in complex autoimmune presentations.
    • The findings contribute to understanding the mechanisms of hypocomplementemia in vasculitic syndromes.
