    Choroid Plexus Neoplasms: Toward a Distinction between Carcinoma and Papilloma Using Arterial Spin-Labeling.

    Background:

    • Primary hyperparathyroidism (HPT) is a condition characterized by excessive parathyroid hormone secretion.
    • Neurological manifestations of HPT are diverse and can mimic other central nervous system disorders.

    Observation:

    • Two patients presented with distinct neurological symptoms including ataxia, ophthalmoplegia, muscle weakness, atrophy, fasciculations, dysarthria, and dysphagia.
    • Both patients exhibited elevated serum calcium and parathyroid hormone levels, indicative of HPT.

    Findings:

    • Surgical removal of parathyroid adenoma/hyperplastic glands led to HPT remission but did not reverse existing neurological damage.
    • Postmortem examination of one patient revealed amyotrophic lateral sclerosis (ALS)-like pathology.

    Implications:

    • Hyperparathyroidism should be considered in the differential diagnosis of unexplained neurological signs and symptoms.
    • Early detection and management of HPT may be vital for potentially mitigating or preventing severe CNS disease progression.
