    Background:

    • Chronic lymphocytic leukemia (CLL) is a mature B-cell malignancy.
    • Orbital involvement is an uncommon manifestation of CLL.

    Observation:

    • A 68-year-old male presented with bilateral exophthalmos.
    • Diagnosis was chronic lymphocytic leukemia (CLL) based on peripheral lymphocytosis.
    • No lymphadenopathy or organ infiltration was initially detected.

    Findings:

    • Massive orbital infiltration by CLL caused the exophthalmos.
    • The patient showed an excellent response to radiotherapy.
    • Orbital involvement as an early feature of CLL is rare.

    Implications:

    • Orbital involvement should be considered in the differential diagnosis of adult bilateral exophthalmos.
    • Early recognition and treatment of orbital CLL can lead to favorable outcomes.
    • This case underscores the diverse clinical presentations of chronic lymphocytic leukemia.
