    Background:

    • Hairy cell leukemia (HCL) is a rare B-cell chronic lymphoid leukemia.
    • Clinical presentation of HCL can vary, impacting diagnosis and prognosis.

    Purpose of the Study:

    • To review the clinical course of HCL patients.
    • To identify factors associated with disease presentation and outcomes.

    Main Methods:

    • Retrospective review of 31 consecutive HCL patients.
    • Classification of patients into typical and atypical disease groups based on splenomegaly and marrow cellularity.

    Main Results:

    • Atypical HCL patients exhibited milder disease and less anemia.
    • Treatment with corticosteroids or cytotoxic agents significantly increased serious infection risk (66%) and mortality (50%).
    • Chemotherapy and age over 50 were key predictors of mortality.

    Conclusions:

    • Atypical HCL may present with less severe anemia.
    • Aggressive treatments in HCL patients, particularly those who are neutropenic, carry substantial risks of serious infections and mortality.
    • Long-term survival is possible, with an excellent prognosis for patients surviving two years.
