    Summary

    Many diabetic patients show abnormal liver function tests (LFTs), particularly non-insulin-dependent individuals. Overweight, poor diabetes control, and older age at onset are linked to abnormal alanine aminotransferase (Alt) levels.

    Background:

    • Diabetes mellitus is associated with various complications, including liver abnormalities.
    • Liver function tests (LFTs) are commonly used to assess liver health.
    • The relationship between diabetes and LFTs requires further investigation.

    Purpose of the Study:

    • To evaluate the prevalence of abnormal LFTs in diabetic outpatients.
    • To explore the association between LFT results and liver histology in diabetic inpatients.
    • To identify clinical factors associated with abnormal LFTs in diabetes.

    Main Methods:

    • Assessed nine LFTs in 175 diabetic outpatients on various treatments.
    • Investigated LFTs against liver biopsy findings in 72 diabetic inpatients.
    • Utilized multivariate analysis to identify predictors of abnormal alanine aminotransferase (Alt).

    Main Results:

    • 57% of outpatients had at least one abnormal LFT; 27% had two abnormal tests.
    • Non-insulin-dependent diabetics showed more abnormal LFTs than insulin-dependent.
    • Abnormalities in Alt and gamma-glutamyl transpeptidase (gGT) increased with histologic severity, but mean values did not significantly differ.
    • Overweight, poor glycemic control, and older age at onset were linked to abnormal Alt in outpatients.

    Conclusions:

    • Abnormal LFTs are common in diabetic patients, especially those with non-insulin-dependent diabetes.
    • While some LFTs correlate with liver histology severity, they may not always reflect significant changes.
    • Clinical factors like obesity and glycemic control are important considerations for interpreting LFTs in diabetics.
