    Summary

Measurement methods significantly impact QT interval assessments. A rigorous method, minimizing artifact, revealed uniform repolarization abnormalities in patients with QT prolongation, unlike conventional methods.

    Area of Science:

    • Cardiology
    • Electrophysiology
    • Medical Imaging

    Background:

    • QT interval assessment is crucial for evaluating cardiac repolarization.
    • Variability in QT measurements can arise from different methodologies and interlead differences.
    • Understanding these variations is key to accurate diagnosis of repolarization abnormalities.

    Purpose of the Study:

    • To evaluate the influence of measurement techniques on QT interval determinations.
    • To characterize the spectrum of QT values and interlead variability across the body surface.
    • To compare a conventional method with a more rigorous approach for QT measurement.

    Main Methods:

    • Simultaneously recorded, signal-averaged ECGs from 120 leads were analyzed in 10 normal subjects and 14 patients with QT prolongation.
    • Two QT measurement methods were used: Method A (conventional) and Method B (rigorous, excluding artifact-prone leads).
    • Interlead QT variability was assessed in absolute terms and as a percentage of average body surface values.

    Main Results:

    • Both methods showed significantly higher mean QTc values in patients with QT prolongation compared to normal subjects.
    • Method A revealed greater interlead QTc variability in patients (32%) than in normal subjects (22%).
    • Method B demonstrated similar interlead QTc variability in both groups (18% in patients, 19% in normal subjects), suggesting uniform repolarization abnormalities in patients.

    Conclusions:

    • Measurement methodology critically affects QT determinations and interlead variability.
    • A rigorous QT measurement technique can minimize artifact and reveal uniform repolarization abnormalities in QT prolongation.
    • Significant interlead QT variation exists across the torso, emphasizing the importance of standardized measurement protocols.
