    This study found that while rapid changes didn't correlate, overall increases in intraocular pressure during glucose tolerance tests were linked to higher plasma glucose and osmolality, suggesting fluid balance shifts in the eye.

    Background:

    • Glaucoma is a leading cause of irreversible blindness.
    • Understanding factors influencing intraocular pressure (IOP) is crucial for glaucoma management.
    • Acute hyperglycemia can cause refractive changes, but the underlying mechanisms require clarification.

    Purpose of the Study:

    • To investigate the relationship between plasma glucose levels, plasma osmolality, and intraocular pressure during a glucose tolerance test in glaucoma patients.
    • To elucidate the role of fluid balance adjustments in refractive changes associated with hyperglycemia.

    Main Methods:

    • A standard glucose tolerance test was administered to 54 patients with chronic simple or low-tension glaucoma.
    • Intraocular pressure and plasma glucose levels were measured throughout the test.
    • Plasma osmolality was measured in a subset of 12 patients.

    Main Results:

    • No significant correlations were found between the rates of change of IOP, plasma glucose, and plasma osmolality.
    • However, the overall increase in intraocular pressure correlated with the overall increase in plasma glucose.
    • The overall increase in plasma glucose also correlated with the overall increase in plasma osmolality.

    Conclusions:

    • Acute hyperglycemia induces changes in refraction through adjustments in fluid balance between intraocular compartments.
    • These findings highlight the complex interplay between metabolic status and ocular fluid dynamics in glaucoma patients.
