    A novel skin flap technique offers robust coverage for ankle and heel defects. This arterially based, innervated flap provides excellent wound healing with minimal donor site issues.

    Findings:

    • Successful repair of defects around the ankle and heel in nine patients.
    • The flap provided excellent, well-bedded cover.
    • The surgical procedure was short.
    • Donor site problems were insignificant.

    Implications:

    • This technique offers a reliable solution for complex lower extremity reconstructions.
    • It minimizes donor site morbidity, improving patient outcomes.
    • Further studies may explore its application in larger defect sizes.
