    Summary

    Electroconvulsive therapy (ECT) affects prolactin levels differently based on stimulation type. Bilateral ECT significantly increases serum prolactin more than unilateral ECT, suggesting greater hypothalamic stimulation.

    Background:

    • Electroconvulsive therapy (ECT) is a treatment for severe mental health conditions.
    • The neuroendocrine effects of ECT, particularly prolactin release, are of clinical interest.

    Purpose of the Study:

    • To compare post-ictal serum prolactin levels after bilateral versus unilateral ECT.
    • To investigate the hypothalamic-stimulating effects of different ECT stimulation types.

    Main Methods:

    • Serial serum prolactin levels were measured in six patients over one hour post-ECT.
    • Patients received either bilateral or unilateral electroconvulsive therapy.

    Main Results:

    • Bilateral ECT resulted in significantly higher mean post-ictal prolactin levels compared to unilateral ECT.
    • Elevated prolactin levels were observed at 15, 20, 30, and 60 minutes post-ECT for bilateral stimulation.

    Conclusions:

    • Bilateral ECT exhibits a greater hypothalamic-stimulating effect than unilateral ECT.
    • This enhanced stimulation may contribute to the therapeutic benefits of bilateral ECT in melancholic patients.
