Cholelithiasis and colorectal cancer.

M Gafà, L Sarli, G Sgobba

    Summary

Gallstones (cholelithiasis) are linked to a higher risk of colorectal cancer, not gallbladder removal surgery (cholecystectomy). This study suggests gallstones themselves may play a role in colorectal cancer development.

    Gastroenterologie clinique et biologique·2008

    Area of Science:

    • Gastroenterology
    • Epidemiology
    • Oncology

    Background:

    • Cholelithiasis (gallstones) and cholecystectomy (gallbladder removal) are common procedures.
    • Previous studies have suggested a link between cholecystectomy and increased colorectal cancer risk, but the causal relationship remains unclear.

    Purpose of the Study:

    • To investigate whether the increased risk of colorectal cancer is associated with cholelithiasis or cholecystectomy.
    • To differentiate the potential impact of gallstones versus gallbladder surgery on colorectal cancer incidence.

    Main Methods:

    • A retrospective epidemiologic study was conducted.
    • The study included 350 patients who underwent surgery for colorectal cancer.
    • Patient histories of cholecystectomy and the presence of concomitant cholelithiasis were compared to control groups.

    Main Results:

    • Patients with colorectal cancer showed a significantly higher incidence of previous cholecystectomy (relative risk: 1.48) and concomitant cholelithiasis (relative risk: 1.99) compared to controls.
    • The association was more pronounced for colorectal cancer in both cases.
    • The risk associated with cholelithiasis was higher than that associated with cholecystectomy.

    Conclusions:

    • The findings suggest that cholelithiasis, rather than cholecystectomy, is associated with an increased risk of colorectal cancer.
    • Cholelithiasis may be a contributing factor to the development of colorectal cancer, either independently or as the reason for cholecystectomy.
