    Cerebrospinal fluid enolase levels correlate with stroke volume and predict poor patient outcomes. Elevated alpha and gamma enolase indicate a worse prognosis following cerebral infarction.

    Background:

    • Stroke is a leading cause of disability and death.
    • Biomarkers are needed for early diagnosis and prognosis assessment.
    • Enolase, an enzyme, is found in various tissues, including the brain.

    Purpose of the Study:

    • To investigate the relationship between cerebrospinal fluid (CSF) enolase levels and cerebral infarct volume.
    • To determine if CSF enolase levels can predict clinical outcomes after stroke.

    Main Methods:

    • CSF samples were collected within 4 days of stroke onset from 28 patients (2 with transient ischemic attacks, 26 with completed stroke).
    • Alpha and gamma enolase levels in CSF were measured.
    • Cerebral infarct volume was assessed using CT imaging.
    • Clinical outcomes were evaluated.

    Main Results:

    • Elevated CSF enolase levels were observed in most patients with transient ischemic attacks and completed stroke.
    • A positive correlation was found between infarct volume and CSF alpha and gamma enolase levels.
    • High CSF enolase levels were consistently associated with poor patient prognosis.

    • CSF alpha and gamma enolase are potential biomarkers for stroke severity.
