    Summary

This study investigated sustained-release theophylline for chronic bronchitis. While symptom improvement was not observed, significant enhancements in pulmonary function tests were noted.

    Area of Science:

    • Pulmonary Medicine
    • Pharmacology

    Background:

    • Chronic bronchitis is a debilitating respiratory condition.
    • Effective management strategies for chronic bronchitis symptoms and pulmonary function are crucial.

    Purpose of the Study:

    • To evaluate the efficacy of sustained-release theophylline in patients with chronic bronchitis.
    • To assess the impact of theophylline on both subjective symptoms and objective pulmonary function.

    Main Methods:

    • A double-blind, cross-over study design was employed.
    • Patients received escalating doses of sustained-release theophylline (350 mg and 700 mg daily) or placebo.
    • Serum theophylline concentrations and pulmonary function parameters were measured.

    Main Results:

    • Theophylline achieved therapeutic serum concentrations (mean 13.9 µg/ml).
    • No significant improvement in reported symptoms was observed.
    • Statistically significant improvements were recorded in Peak Expiratory Flow Rate (PEFR), 1-second forced expiratory volume (FEV1), and forced vital capacity (FVC).

    Conclusions:

    • Sustained-release theophylline may offer benefits in improving pulmonary mechanics in chronic bronchitis patients.
    • Further research is warranted to explore its role in managing this condition, considering symptom perception versus objective measures.
