Rheumatoid arthritis (RA) patients exhibit defective monocyte bactericidal activity, increasing infection susceptibility. Phagocytosis and antibody-dependent cellular cytotoxicity (ADCC) were unaffected, suggesting specific immune defects in RA.

    Area of Science:

    • Immunology
    • Rheumatology

    Background:

    • Rheumatoid arthritis (RA) is a chronic inflammatory disease.
    • Patients with RA often experience increased susceptibility to infections.

    Purpose of the Study:

    • To investigate the functional capacity of monocytes from RA patients.
    • To determine if defects in monocyte function correlate with RA disease activity or infection susceptibility.

    Main Methods:

    • Monocytes were isolated from peripheral blood of RA patients and healthy controls.
    • Bactericidal activity, phagocytosis, and antibody-dependent cellular cytotoxicity (ADCC) were assessed.
    • The effect of RA patient serum on normal monocyte ADCC was evaluated.

    Main Results:

    • Monocytes from RA patients showed a marked defect in bactericidal activity against Staphylococcus albus and Listeria monocytogenes.

    Related Experiment Videos

  • Phagocytic capacity of RA monocytes was comparable to controls for Staphylococcus albus and Shigella flexneri.
  • No significant difference in monocyte ADCC was observed between RA patients and controls.
  • Incubation with RA patient serum markedly reduced the ADCC of normal monocytes.
  • No correlation was found between rheumatoid factor (RF) titer and monocyte ADCC in RA patients.
  • Conclusions:

    • A defect in monocyte bactericidal activity may contribute to the increased infection susceptibility in RA patients.
    • While phagocytosis and ADCC appear normal, specific bactericidal defects highlight a potential mechanism for impaired host defense in RA.
