    Summary

Group B beta-hemolytic streptococcus (GBS) can cause serious adult infections, particularly in those with underlying conditions. Prophylactic penicillin successfully prevented recurrent GBS erysipelas in a patient with chronic lymphedema.

    Area of Science:

    • Infectious Diseases
    • Bacteriology
    • Clinical Medicine

    Background:

    • Group B beta-hemolytic streptococcus (GBS) is a significant pathogen, primarily known for neonatal infections.
    • Serious GBS infections in adults are often associated with predisposing medical conditions.
    • Chronic lymphedema can increase susceptibility to infections.

    Observation:

    • A case of recurrent erysipelas in a woman with chronic lymphedema post-hysterectomy and radiation therapy is presented.
    • Type III GBS was isolated from multiple sites, including vulvar vesicles, vaginal mucosa, and blood.
    • Erysipelas is a bacterial skin infection that can lead to serious complications.

    Findings:

    • The identification of Type III GBS in a recurrent erysipelas case highlights its potential in adult infections.
    • Prophylactic penicillin V potassium therapy proved effective in preventing subsequent erysipelas episodes.
    • This case demonstrates the efficacy of targeted antibiotic prophylaxis in managing recurrent GBS infections.

    Implications:

    • Group B beta-hemolytic streptococcus should be considered in the differential diagnosis of adult erysipelas and cellulitis.
    • Understanding GBS in adults with compromised lymphatic systems is crucial for effective treatment.
    • Prophylactic antibiotic strategies may be beneficial for individuals at high risk of recurrent GBS infections.
