    Summary

    Aspirin absorption via feeding tube in elderly patients showed normal peak serum levels but a prolonged drug half-life. This suggests altered drug metabolism or elimination in patients with feeding jejunostomies.

    Background:

    • Dysphagia paralytica necessitates alternative medication routes, such as feeding jejunostomy.
    • Understanding drug absorption and disposition is crucial for safe and effective geriatric pharmacotherapy.

    Observation:

    • A 67-year-old male with a feeding jejunostomy received 975 mg of aspirin as a slurry.
    • Serum salicylate concentrations were monitored to assess aspirin absorption.

    Findings:

    • Peak serum salicylate levels after jejunostomy administration were comparable to oral administration in previous geriatric studies.
    • The elimination half-life of salicylate was unexpectedly prolonged, averaging 7.5 hours compared to 3.7 hours in prior studies.

    Implications:

    • Jejunal administration of aspirin may lead to altered drug pharmacokinetics in elderly patients.
    • Further research is needed to elucidate the mechanisms behind the prolonged salicylate half-life and its clinical significance.
    • Dosage adjustments or alternative formulations may be considered for aspirin therapy in patients with feeding jejunostomies.
