    Summary

Systemic lupus erythematosus (SLE) can cause optic neuritis, an inflammation of the optic nerve. Prompt corticosteroid treatment may improve vision in patients with SLE-related optic neuritis.

    Area of Science:

    • Ophthalmology
    • Rheumatology
    • Immunology

    Background:

    • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with diverse clinical manifestations.
    • Ocular involvement in SLE can be significant, impacting visual function.

    Observation:

    • A 57-year-old man presented with unilateral optic neuritis.
    • Neurological examination was unremarkable, but laboratory tests revealed elevated inflammatory markers, positive antinuclear antibody (ANA) titer, and LE preparation.
    • Urinalysis showed red blood cell casts.

    Findings:

    • The patient was diagnosed with systemic lupus erythematosus (SLE).
    • Optic neuritis (papillitis) in this case is presumed to be a manifestation of generalized vasculitis associated with SLE.
    • Treatment with corticosteroids resulted in moderate improvement of central visual acuity.

    Implications:

    • This case highlights optic neuritis as a potential presenting or concurrent ocular manifestation of SLE.
    • Early diagnosis and treatment of SLE-related optic neuritis with corticosteroids may lead to visual recovery.
    • Understanding the link between SLE vasculitis and optic nerve inflammation is crucial for managing patients with both conditions.
