    Background:

    • Idiopathic guttate hypomelanosis (IGH) presents as small, acquired white spots on the skin.
    • IGH is often associated with guttate hyperkeratosis, xerosis, and lentiginosis.
    • These clinical features are not consistently related to chronological age.

    Purpose of the Study:

    • To investigate the histological and clinical characteristics of idiopathic guttate hypomelanosis.
    • To explore the relationship between hypopigmented macules and associated hyperkeratotic lesions.
    • To determine the underlying pathomechanism of idiopathic guttate hypomelanosis.

    Main Methods:

    • Histological examination of hypopigmented macules and hyperkeratotic lesions.
    • Assessment of melanin content, DOPA-oxidase activity, and melanosome stages in melanocytes.
    • Clinical evaluation including assessment of associated skin conditions.

    Main Results:

    • Hypopigmented macules exhibit significantly decreased melanin, reduced DOPA-oxidase activity, and fewer, smaller melanosomes (Stages I & II) in melanocytes.

    • Idiopathic guttate hypomelanosis is strongly associated with an early skin aging process.
    • The observed histological changes in melanocytes and epidermis support the theory of premature aging.
    • IGH may represent a manifestation of accelerated photoaging or intrinsic aging of the skin.
