    Summary

    The presence of an A wave on a mitral echogram in patients with normal sinus rhythm can help exclude severe mitral stenosis. However, its absence does not definitively indicate the severity of mitral stenosis.

    Background:

    • Mitral stenosis is a significant valvular heart disease.
    • Accurate assessment of mitral stenosis severity is crucial for patient management.
    • Echocardiography is a primary tool for evaluating mitral valve function.

    Purpose of the Study:

    • To determine if the presence or absence of an A wave on mitral echograms can predict mild versus severe mitral stenosis.
    • To correlate mitral echogram findings with mitral valve area (MVA) determined by the Gorlin formula.

    Main Methods:

    • M-mode echocardiograms were analyzed in 35 patients with catheterization-proven mitral stenosis and normal sinus rhythm.
    • Mitral valve area (MVA) was calculated using the Gorlin formula.
    • Presence of a mitral A wave was defined as ≥2 mm anterior motion of the anterior mitral leaflet.

    Main Results:

    • 16 patients without a definite A wave had a mean MVA of 1.18 cm².
    • 13 patients with a definite A wave had a mean MVA of 2.04 cm².
    • A significant difference (p < 0.001) in mean MVA was observed between groups; no patient with a definite A wave had MVA < 1.2 cm².

    Conclusions:

    • A definite A wave on mitral echogram in sinus rhythm excludes severe mitral stenosis.
    • The absence of an A wave does not allow for definitive conclusions regarding mitral stenosis severity.
    • Echocardiographic A wave assessment can aid in the non-invasive evaluation of mitral stenosis.
