    Papillary adenocarcinoma in a median ectopic thyroid is rare. Local excision with the Sistrunk procedure is recommended for this thyroid cancer, with potential benefits from thyroid hormone suppression.

    • Papillary adenocarcinoma of the median ectopic thyroid is uncommon.
    • Local excision combined with the Sistrunk procedure is the recommended surgical approach.
    • Thyroid hormone suppression therapy may offer adjunctive benefits.

    Implications:

    • Early diagnosis and appropriate surgical management are crucial for papillary adenocarcinoma of the median ectopic thyroid.
