    Background:

    • Multiple myeloma is a plasma cell malignancy.
    • Peripheral neuropathy is a known complication, but its pathogenesis remains unclear.
    • Light chain K-type myeloma is a specific subtype.

    Observation:

    • An adult female presented with sensorimotor polyneuropathy.
    • Neurophysiological tests revealed reduced peripheral nerve conduction velocity.
    • Sural nerve biopsy showed significant loss of myelinated and unmyelinated fibers.
    • Electron microscopy ruled out amyloid deposits but revealed unusual Schwann cell proliferation around demyelinated fibers and collagen pockets.

    Findings:

    • The patient had adult-onset sensorimotor polyneuropathy associated with light chain K-type myeloma.
    • Nerve biopsy demonstrated severe fiber loss and demyelination without amyloid.
    • Evidence of active remyelination and collagen accumulation was observed.

    Implications:

    • The findings suggest a potential role for specific immunoglobulins in the pathogenesis of polyneuropathy in light chain myeloma.
    • This case highlights an unusual presentation and potential mechanism for nerve damage in multiple myeloma.
    • Further research into immunoglobulin-mediated neuropathy in myeloma is warranted.
