    Area of Science:

    • Psychiatry
    • Clinical Psychology
    • Mental Health Research

    Background:

    • Chronic schizophrenia is a complex condition affecting long-stay patients.
    • Understanding symptom presentation in non-paranoid schizophrenia is crucial for treatment.
    • The hierarchy of classes of personal illness model provides a framework for symptom analysis.

    Purpose of the Study:

    • To investigate symptom patterns in chronic non-paranoid schizophrenic patients using the Delusions-Symptoms-States Inventory (DSSI).
    • To compare symptom distribution in chronic patients with that of acutely ill patients.
    • To identify the prevalence of delusions, affective states, and depressive symptoms in this population.

    Main Methods:

    • The Delusions-Symptoms-States Inventory (DSSI) was administered to 33 chronic non-paranoid schizophrenic patients.
    • Patients were classified according to the hierarchy of classes of personal illness model.
    • Symptom items were analyzed both within the model and independently.

    Main Results:

    • 81% of patients' symptom patterns conformed to the hierarchy of classes of personal illness model.
    • Symptom distribution differed significantly from that observed in acutely ill patients.
    • Half of the patients fell into the lowest classes (Class 0: Symptom-free, Class 1: Dysthymic States).
    • 50% of patients reported recent delusions.
    • Over a third reported affective states and depressive symptoms.

    Conclusions:

    • Chronic non-paranoid schizophrenic patients on long-stay wards show distinct symptom profiles compared to acute cases.
    • A significant portion of chronic patients may present with delusions and affective/depressive symptoms, even within lower illness classes.
    • The DSSI is a valuable tool for characterizing symptom states in chronic schizophrenia.
