    Background:

    • The micronodular pulmonary pattern presents as small, homogeneous densities (<1mm) without reticular features.
    • Differential diagnoses include alveolar microlithiasis and intravenous talc injection.

    Observation:

    • Two patients with this pattern had significant hairspray exposure and noncaseating granulomatous lung reactions.
    • Review of 31 cases revealed the micronodular pattern in eight patients exposed to hairspray aerosols.

    Findings:

    • Electron microscopy and x-ray spectrometry analyzed various aerosol sprays.
    • The exact pathogenesis of hairspray-induced pulmonary reactions remains undetermined.

    Implications:

    • Hairspray aerosol exposure is a potential cause of the micronodular pulmonary pattern.
    • Corticosteroid therapy and cessation of aerosol use may resolve the condition.
