    Summary

    This study quantifies gonadotropin-releasing hormone (GnRH) pulse amplitude changes during puberty in children. Early pubertal responses suggest nocturnal GnRH pulse amplitude equals or exceeds that in adult men.

    Background:

    • Puberty involves complex hormonal regulation.
    • Gonadotropin-releasing hormone (GnRH) plays a crucial role in initiating pubertal development.
    • Understanding GnRH pulse dynamics is key to deciphering pubertal initiation.

    Purpose of the Study:

    • To quantitatively estimate changes in GnRH pulse amplitude in hypophysial portal plasma during human puberty.
    • To compare induced gonadotropin responses to synthetic GnRH with spontaneous fluctuations.
    • To investigate GnRH's role in initiating the pubertal surge of luteinizing hormone (LH).

    Main Methods:

    • Studied 14 children aged 2.5–16 years.
    • Administered low-dose synthetic GnRH pulses intravenously every 2 hours.
    • Measured induced luteinizing hormone (LH) and follicle-stimulating hormone (FSH) responses, comparing them to spontaneous LH and FSH fluctuations.

    Main Results:

    • Maximal peripheral plasma GnRH concentrations varied with dosage (107 ± 25 pg/ml for 0.0125 µg/kg, 218 ± 33 pg/ml for 0.025 µg/kg).
    • In early puberty, induced LH responses were similar to or less than spontaneous nocturnal LH surges.
    • LH responses were minimal in children with bone ages under 10 years; GnRH injections did not mimic the nocturnal LH rise.

    Conclusions:

    • The amplitude of nocturnal GnRH pulses in early pubertal children appears to be equal to or greater than that observed in adult males.
    • GnRH pulse amplitude is a critical factor in initiating pubertal gonadotropin secretion.
    • Further research is needed to fully elucidate the GnRH control of puberty.
