    Background:

    • Rh immunisation poses a significant risk during pregnancy, potentially leading to hemolytic disease of the fetus and newborn (HDFN).
    • Monitoring fetal well-being is crucial in Rh-immunized pregnancies to guide timely interventions.

    Observation:

    • Amniocentesis was performed in 42 pregnant women with Rh immunisation (56 procedures).
    • Liley diagram assessment identified high delta bilirubin extinction in the B zone (6 cases) and A zone (16 cases).

    Findings:

    • In cases with high delta bilirubin extinction in the B zone, immunosuppressive therapy with corticosteroids was administered.
    • All cases with high delta bilirubin extinction in the B zone and those in the A zone ultimately required neonatal exsanguinotransfusion.

    Implications:

    • This study highlights the critical role of amniocentesis and Liley diagram interpretation in managing Rh immunisation.
    • Aggressive management, including corticosteroids and neonatal exchange transfusion, is often necessary for severe cases of HDFN.
