    Background:

    • Pneumococcal disease remains a significant cause of mortality, even with available antimicrobial treatments.
    • Understanding the host immune response is crucial for predicting patient outcomes in pneumococcal infections.

    Purpose of the Study:

    • To investigate the correlation between specific immunologic markers and survival in patients with pneumococcal disease.
    • To compare immune profiles of patients who survived versus those who died from pneumococcal infection.

    Main Methods:

    • Serum samples from 18 pneumococcal patients (5 fatal, 13 survivors) were analyzed.
    • Assays included detection of pneumococcal capsular polysaccharide (PCP) antigen (CIE), PCP antibody (RIA), and complement components (C3, C4, C3PA) and immunoglobulins (IgG, IgM, IgA) (radial immunodiffusion).

    Main Results:

    • Higher PCP antigen levels (≥15 µg/ml) were significantly associated with fatal outcomes (p=0.022).
    • Lower levels of specific antibodies to PCP were observed in non-survivors (p=0.027).
    • Decreased complement components were prevalent in non-survivors upon admission (p<0.01) and persisted until death, unlike survivors.

    Conclusions:

    • Serum pneumococcal capsular polysaccharide antigen levels and complement component status are critical indicators of prognosis in pneumococcal disease.
    • Immunologic profiling can aid in identifying high-risk patients requiring intensive management.
