    Summary

    Chronic hypophosphatemia in adults can cause osteopenia due to reduced bone formation, not always osteomalacia. This study explored this unusual bone disease in patients with renal phosphate wasting.

    Background:

    • Chronic hypophosphatemia is often associated with renal phosphate wasting.
    • Osteomalacia is a common consequence, but other bone pathologies can occur.

    Purpose of the Study:

    • To investigate bone histomorphometry in adults with chronic hypophosphatemia without histological osteomalacia.
    • To characterize the bone disease associated with idiopathic renal phosphate wasting.

    Main Methods:

    • Bone histomorphometry was performed on 19 adult patients with chronic hypophosphatemia.
    • Patients were grouped based on clinical presentation: renal lithiasis, radiological osteoporosis, or lumbar pain.
    • Serum phosphate, blood calcium, urine calcium, and intact parathyroid hormone (iPTH) levels were measured.

    • Patients exhibited low serum phosphate, normal blood calcium, increased urine calcium, and low iPTH.
    • Histomorphometry revealed decreased osteoblastic surfaces with normal resorption and mineralization.

    • Chronic hypophosphatemia in adults can lead to an osteopathy characterized by osteopenia from decreased bone formation, rather than solely osteomalacia.
    • The roles of phosphate deficiency and low iPTH in this condition's pathogenesis require further investigation.
