    Observation:

    • A 60-year-old woman presented with recurrent anterior uveitis in her right eye.
    • Initial corticosteroid treatment resulted in temporary improvement, followed by severe relapse, endophthalmitis, and blindness.

    Findings:

    • Pulmonary tuberculosis was diagnosed during the relapse.
    • Histologic examination of the enucleated eye revealed numerous acid-fast bacilli.

    Implications:

    • Tuberculosis should be considered in the differential diagnosis of unexplained or recurrent uveitis, especially in endemic areas.
    • Early detection and systemic treatment of tuberculosis are crucial to prevent severe ocular complications and vision loss.
