    Background:

    • Bilateral total hip replacement (BTHR) can be performed in various surgical configurations.
    • Comparing single-stage versus staged BTHR is crucial for optimizing patient care and resource utilization.

    Purpose of the Study:

    • To compare the outcomes of single-stage bilateral total hip replacement (BTHR) with staged BTHR performed during the same or separate hospitalizations.

    Main Methods:

    • A retrospective review of 122 patients undergoing single-stage BTHR.
    • Comparison with two control groups: staged BTHR in the same hospitalization and staged BTHR in separate hospitalizations.

    Main Results:

    • Single-stage BTHR showed approximately doubled surgical duration and one-third increased blood loss compared to single procedures.
    • Length of stay was reduced by approximately half for single-stage BTHR versus two-stage, single-admission replacements.

    • Careful patient selection is warranted due to a reported death from fat embolus in the single-stage group.
