Tuberculous lymphadenitis in Singapore.

Y C Chee

Annals of the Academy of Medicine, Singapore
    |October 1, 1982

    Summary

Tuberculous lymphadenitis is common in young females, particularly Indians and Malays. Mycobacterial culture of lymph node biopsies offers a more sensitive and specific diagnosis than histology for tuberculosis.

    Area of Science:

    • Infectious Diseases
    • Microbiology
    • Pathology

    Background:

    • Tuberculous lymphadenitis is the second most common form of tuberculosis.
    • Diagnosis relies on lymph node biopsy, histology, and bacteriology.
    • Previous studies noted female preponderance and higher incidence in certain ethnic groups.

    Purpose of the Study:

    • To analyze the pattern of tuberculous lymphadenitis.
    • To evaluate diagnostic methods for tuberculous lymphadenitis.
    • To compare histological and bacteriological diagnostic rates.

    Main Methods:

    • Retrospective analysis of 97 patients diagnosed with tuberculous lymphadenitis.
    • Lymph node biopsy with histological examination and mycobacterial culture.
    • Comparison of culture-positive rates versus histological identification of acid-fast bacilli.

    Main Results:

    • Tuberculous lymphadenitis predominantly affects young individuals (over 60% under 31) and involves the neck (88.6%).
    • A female preponderance and disproportionate incidence in Indian and Malay populations were observed.
    • Mycobacterial culture yielded a 64% positive rate for Mycobacterium tuberculosis, significantly higher than histological detection (13.4%).

    Conclusions:

    • Mycobacterial culture is more sensitive and specific for diagnosing tuberculous lymphadenitis than histology.
    • Positive cultures confirm tuberculosis, identify mycobacteria, and enable drug sensitivity testing.
    • Proper sample handling (saline, not formalin, for culture) is crucial for accurate diagnosis.
