Endotoxaemia following colonoscopy.

A Kiss, P Ferenci, W Graninger

    Endoscopy

    Summary

Colonoscopy frequently causes endotoxaemia (endotoxin in the blood), but not bacteraemia (bacteria in the blood). This condition, particularly in liver cirrhosis patients, requires further investigation for clinical implications.

    Area of Science:

    • Gastroenterology
    • Infectious Diseases
    • Clinical Microbiology

    Background:

    • Colonoscopy is a common diagnostic procedure for evaluating the colon.
    • Potential complications include the translocation of bacterial products into the bloodstream.

    Purpose of the Study:

    • To investigate the incidence of bacteraemia and endotoxaemia following colonoscopy.
    • To identify factors associated with post-colonoscopy bacteraemia and endotoxaemia.
    • To assess the clinical significance of these findings, especially in patients with liver disease.

    Main Methods:

    • Prospective study of 100 consecutive patients undergoing colonoscopy.
    • Blood samples analyzed for bacteraemia and endotoxaemia using the Limulus Amoebocyte Lysate Test.
    • Data collected on procedure duration, biopsies, bowel preparation, and patient liver status.

    Main Results:

    • No bacteraemia was detected post-colonoscopy; Staph. epidermidis growth was attributed to contamination.
    • Endotoxaemia occurred in 9% of patients.
    • Endotoxaemia incidence was not linked to procedure length, biopsy number, or bowel cleansing, but was higher in patients with liver cirrhosis.

    Conclusions:

    • Colonoscopy is frequently followed by endotoxaemia.
    • Liver cirrhosis is a risk factor for post-colonoscopy endotoxaemia.
    • The clinical implications of endotoxaemia after colonoscopy, particularly in patients with liver cirrhosis, warrant further study.
