Vulvar carcinoma in situ.

S G Bernstein, B R Kovacs, D E Townsend

    Obstetrics and Gynecology

    Summary

This study reviewed vulvar carcinoma in situ (CIS) in 65 patients, focusing on lesion location and patient demographics. Younger patients (<40) often had multifocal CIS, while older patients (>40) typically presented with unifocal disease.

    Area of Science:

    • Gynecologic Oncology
    • Dermatopathology

    Background:

    • Carcinoma in situ of the vulva (VIN 3) is a premalignant condition.
    • Understanding its epidemiology and lesion characteristics is crucial for effective management.

    Purpose of the Study:

    • To analyze the epidemiology, symptom duration, and precise anatomical locations of vulvar carcinoma in situ (CIS) lesions.
    • To identify patterns in lesion distribution and multifocality based on patient age.

    Main Methods:

    • Retrospective review of 65 patients diagnosed with vulvar CIS.
    • Diagrammatic division of the vulva and perineum into 16 geographic compartments.
    • Calculation of lesion location frequency within each compartment.

    Main Results:

    • The inferior border of the right labium majus was the most frequent site for vulvar CIS.
    • Eighty-four percent of patients under 40 years old presented with multifocal disease.
    • Sixty-five percent of patients over 40 years old had unifocal disease.

    Conclusions:

    • Lesion location in vulvar CIS shows a predilection for the inferior right labium majus.
    • Age is a significant factor differentiating multifocal versus unifocal presentation of vulvar CIS.
    • These findings can inform diagnostic and treatment strategies for vulvar intraepithelial neoplasia.
