    High-dose isosorbide dinitrate (480 mg/d) effectively reduced angina attacks in patients unresponsive to lower doses. Most patients tolerated the high dosage long-term, showing improved well-being and reduced nitroglycerin use.

    Background:

    • Stable angina pectoris (grade 3) often requires effective treatment.
    • Low-dose isosorbide dinitrate may not be sufficient for all patients.
    • Alternative treatment options are needed for non-responders.

    Purpose of the Study:

    • To evaluate the efficacy and tolerability of high-dose isosorbide dinitrate (480 mg/d) in patients with stable angina.
    • To assess long-term responsiveness and safety of high-dose isosorbide dinitrate.
    • To determine if high-dose isosorbide dinitrate can benefit patients unresponsive to low doses.

    Main Methods:

    • Single-blind trial involving 24 patients with grade 3 stable angina pectoris.
    • Patients received placebo for 4 weeks, followed by increasing doses of isosorbide dinitrate for 6 weeks.
    • Responders and tolerators continued high-dose (480 mg/d) treatment for an average of 1 year.

    Main Results:

    • Average weekly angina attacks decreased by 74% (from 6.05 to 1.6) with high-dose treatment (p < 0.01).
    • Nitroglycerin consumption decreased, and most patients reported improved activity, well-being, and angina thresholds.
    • Long-term treatment (avg. 1 year) showed stable or downward trends in angina frequency for 18 out of 19 patients.

    Conclusions:

    • High-dose isosorbide dinitrate (480 mg/d) is effective for selected angina patients unresponsive to low doses.
    • This high dosage is generally well-tolerated long-term, offering clinical utility in coronary heart disease management.
    • Isosorbide dinitrate may be a valuable option even when exercise performance shows no significant improvement.
