    Summary

    A new micro-explosion lithotripsy technique successfully crushed large bladder stones using an explosive catheter. Fragments were then removed, demonstrating a promising new method for treating calculi.

    Background:

    • Bladder stones (cystolithiasis) can be challenging to treat, especially large ones.
    • Existing lithotripsy methods may have limitations in fragmenting large calculi.
    • A novel approach using controlled micro-explosions was investigated.

    Observation:

    • A specialized cystolithotriptor with an explosive catheter tip (5 mg lead azide) was developed.
    • The device was used in three clinical cases involving bladder stones ranging from 32 to 65 mm.
    • 1 to 8 micro-explosions were sufficient to fragment the stones.

    Findings:

    • Successful fragmentation of large bladder stones into smaller pieces was achieved in all three cases.
    • The generated fragments were further reduced to grains using Young's cystoscope.
    • Fragments were effectively removed via aspiration post-fragmentation.

    Implications:

    • Micro-explosion lithotripsy shows significant promise as an effective treatment for large bladder stones.
    • This technique could potentially be adapted for treating calculi in other organs in the future.
    • Further research may establish this as a viable alternative to current lithotripsy procedures.
