    Summary

    The new nonionic iohexol contrast agent showed fewer allergic reactions than the commonly used metrizoate during cranial CT scans. Both agents demonstrated similar enhancement efficiency in this randomized study.

    Background:

    • Contrast media are essential for enhancing diagnostic accuracy in cranial computed tomography (CT).
    • Metrizoate (Isopaque Cerebral) is a widely used ionic contrast agent.
    • Iohexol is a newer, nonionic contrast agent with a potentially improved safety profile.

    Purpose of the Study:

    • To compare the safety and efficacy of iohexol versus metrizoate for contrast enhancement in cranial CT.
    • To evaluate the incidence of adverse reactions associated with each contrast medium.

    Main Methods:

    • A randomized, double-blind study was conducted with 210 patients undergoing cranial CT.
    • Patients were randomly assigned to receive either intravenous metrizoate (n=105) or iohexol (n=105).
    • Enhancement efficiency and adverse reactions were systematically recorded.

    Main Results:

    • No serious adverse reactions were reported for either contrast medium.
    • Ten patients (9.5%) receiving metrizoate experienced minor, primarily allergic-type reactions.
    • Only one patient (0.95%) receiving iohexol reported a reaction.
    • No significant differences in cranial CT enhancement efficiency were observed between the two groups.

    Conclusions:

    • Iohexol offers a favorable safety profile with a significantly lower incidence of minor adverse reactions compared to metrizoate for cranial CT.
    • Both metrizoate and iohexol provide comparable enhancement efficiency for cranial CT.
    • Iohexol represents a viable alternative to metrizoate, particularly for patients with potential sensitivities.
