    Summary

    Pyridoxine-dependent seizures are caused by a high vitamin B6 need in the brain. Early vitamin B6 treatment controls seizures, preventing permanent neurological harm in infants.

    Background:

    • Pyridoxine-dependent epilepsy (PDE) is a rare genetic disorder.
    • It arises from an increased requirement for vitamin B6 (pyridoxine) in the central nervous system.

    Observation:

    • Infants with PDE present with seizures that are difficult to control.
    • These seizures are refractory to standard antiepileptic drugs.

    Findings:

    • Prompt administration of pharmacologic doses of vitamin B6 can completely control seizures in affected infants.
    • This intervention is crucial for preventing further neurological damage.

    Implications:

    • PDE should be strongly considered in the differential diagnosis of any infant presenting with intractable seizures.
    • Early diagnosis and treatment with vitamin B6 are essential for favorable neurodevelopmental outcomes.
