    Summary

    Background:

    • The early ventilatory status of patients after head injury is not well understood.
    • Understanding early respiratory changes is vital for timely intervention in traumatic brain injury.

    Purpose of the Study:

    • To investigate the ventilatory status within two hours of head injury.
    • To determine trends toward hypo- or hyperventilation based on head injury severity.

    Main Methods:

    • Prospective study of 63 patients with varying head injury severity at an urban trauma center.
    • Arterial blood gas analysis performed upon emergency presentation.
    • Patients categorized by Glasgow Coma Scale (GCS) scores.

    Main Results:

    • Severe head injury (GCS ≤ 4) patients exhibited mean pH 7.29 and PaCO2 41.86 torr (acidosis and hypercapnia).
    • Moderate (GCS 5-11) and mild (GCS ≥ 12) head injury groups showed less severe acidosis and hypercapnia.
    • Statistically significant differences in pH and PaCO2 were observed between severe and mild head injury groups (P ≤ .01 for pH, P = .05 for PaCO2).

    Conclusions:

    • Patients with severe head trauma demonstrate an early tendency towards hypercapnia and acidosis.
    • Early airway control and assisted ventilation are necessary to optimize PaCO2 levels in severe head injury.
    • Findings highlight the importance of monitoring and managing ventilation in the acute phase of severe head injury.
