    Background:

    • Gastric polyps are classified as adenomatous or hyperplastic/regenerative.
    • Accurate classification and incidence data are limited by terminology and histologic inconsistencies.
    • Polyp size is a criterion used to predict malignant potential.

    Observation:

    • Four patients presented with large, sometimes multiple, gastric masses.
    • These masses were histologically confirmed as benign hyperplastic polyps.
    • None of the large hyperplastic polyps were associated with gastric carcinoma.

    Findings:

    • Large gastric polyps can be benign hyperplastic lesions.
    • Radiographic and pathologic features of these large polyps were reviewed.
    • Benign hyperplastic polyps may not correlate with malignant potential based on size alone.

    Implications:

    • Re-evaluation of size as a sole predictor of malignancy in gastric polyps is warranted.
    • Accurate histologic classification is crucial for understanding gastric polyp behavior.
    • This finding may influence diagnostic and management strategies for large gastric polyps.
