    Background:

    • Floppy mitral valve (FMV) is a common valvular condition.
    • Ventricular septal defect (VSD) is a frequent congenital heart anomaly.
    • The relationship between VSD and FMV requires further elucidation.

    Purpose of the Study:

    • To investigate the incidence of FMV in hearts with isolated VSD.
    • To determine the age-related prevalence and anatomic basis of FMV.
    • To explore potential shared etiologies between VSD and FMV.

    Main Methods:

    • Retrospective analysis of 18% of heart specimens with isolated VSD.
    • Categorization of specimens into three age groups: <1 year, 1-16 years, and 17-91 years.
    • Histopathological examination to assess spongiosal invasion and fibrosa disruption.

    Main Results:

    • FMV was present in 18% of VSD specimens, with no significant age-related difference.
    • FMV was not identified as the cause of death in any case.
    • Associated valve abnormalities included tricuspid (63%), pulmonary (16%), and aortic (5%) valve floppiness.
    • Spongiosal invasion, the proposed anatomic basis, was fully developed by 3 months of age and did not progress with age.
    • No evidence suggests a shared etiology between VSD and FMV.

    Conclusions:

    • FMV is a congenital lesion with full anatomic expression by infancy.
    • FMV in the context of VSD does not appear to share a common cause.
    • FMV is generally not a direct cause of mortality, though complications can occur.
