Postresection bronchopleural fistula: detection by regional ventilation-perfusion studies.

Summary

Diagnosing bronchopleural fistula after lung cancer surgery can be challenging. Xenon 133 ventilation-perfusion scans effectively confirmed this complication in postoperative patients when other methods were inconclusive.

Background:

Bronchopleural fistula (BPF) is a serious complication following pneumonectomy for lung cancer.
Accurate and timely diagnosis of BPF is crucial for patient management and outcomes.
Traditional diagnostic methods, including imaging and clinical assessment, can be inconclusive in early postoperative stages.

Observation:

Three patients developed BPF after right pneumonectomy for bronchogenic carcinoma.
Radiologic and clinical evaluations provided inconclusive evidence for BPF in these cases.
Symptoms and signs of BPF were not definitive in the immediate and late postoperative periods.

Findings:

Regional ventilation-perfusion (V/Q) studies using Xenon 133 were performed.

Xenon 133 V/Q scans demonstrated clear evidence of BPF.

The diagnostic utility of V/Q scanning was confirmed in both immediate and late postoperative assessments.

Implications:

Xenon 133 V/Q scintigraphy is a valuable tool for diagnosing postoperative bronchopleural fistulas.
This imaging modality can aid in definitive diagnosis when conventional methods fail.
Improved diagnostic accuracy for BPF can lead to earlier therapeutic interventions and better patient prognoses.
