    Summary

    A persistent corneal ulcer caused by Mycobacterium chelonei resisted antibiotic treatment. Surgical intervention, specifically penetrating keratoplasty, was necessary for a cure.

    Background:

    • Corneal ulcers can be caused by atypical mycobacteria.
    • Mycobacterium chelonei is an opportunistic pathogen that can cause severe ocular infections.

    Observation:

    • A patient presented with a dendritiform corneal ulcer.
    • Repeated cultures identified Mycobacterium chelonei as the causative agent.

    Findings:

    • The ulcer was resistant to standard topical antibiotic therapies.
    • The organism showed sensitivity only to kanamycin and amikacin in vitro.
    • Topical administration of kanamycin and amikacin did not resolve the infection.

    Implications:

    • Corneal Mycobacterium chelonei infections may require aggressive treatment strategies.
    • Surgical intervention, such as penetrating keratoplasty, can be curative when medical management fails.
    • This case highlights the challenges in treating refractory mycobacterial keratitis.
