    Background:

    • Extrinsic pulmonary granulomatoses (EPG) are lung diseases triggered by inhaled substances.
    • Some EPG cases present with asthma-like symptoms and rhinitis, particularly in individuals without a history of atopy.
    • The underlying immunological mechanisms of these non-classical allergic reactions in EPG require further elucidation.

    Purpose of the Study:

    • To analyze the clinical behavior of EPG patients who developed asthma-like symptoms and rhinitis without prior atopy.
    • To investigate the immunological basis of these EPG manifestations, specifically exploring hypersensitivity reactions.
    • To differentiate these EPG-related respiratory symptoms from classical allergic responses.

    Main Methods:

    • Analysis of 44 EPG patients from a larger cohort (133 cases) with no prior history of atopy.
    • Clinical observation of asthma-like symptoms and rhinitis, including examination of nasal lesions.
    • Bronchial provocation tests performed on 23 patients with suberosis (a type of EPG).
