    Background:

    • A patient developed Cryptococcal endocarditis and myocarditis with a non-regurgitant xenograft aortic prosthesis.
    • New onset of first-degree atrioventricular (A-V) block occurred in the third postoperative week, indicating prosthetic valve dysfunction.

    Findings:

    • Urgent surgical re-intervention with prosthesis replacement and aggressive debridement of the aortic root was performed.
    • The patient experienced a successful outcome, remaining infection-free with a functioning valve one and a half years post-operation.
