    Background:

    • Diuretic medications are frequently prescribed for conditions common in the elderly, such as hypertension.
    • The long-term effects of diuretic use on biochemical parameters in an ambulatory elderly population are not fully characterized.

    Purpose of the Study:

    • To investigate the prevalence of diuretic use in a retirement community.
    • To assess the impact of various diuretics on clinical laboratory values in elderly individuals.

    Main Methods:

    • A cross-sectional study involving 2009 women and 1183 men over 65 years old.
    • Health questionnaires and medication information were collected to identify diuretic users and non-users (control group).
    • Serum electrolyte, uric acid, blood urea nitrogen (BUN), and creatinine levels were compared between groups.

    Main Results:

    • Diuretic use was prevalent (40.2% women, 29.5% men), often long-term and daily.
    • Chlorthalidone and thiazide users showed significant differences in serum potassium; hydrochlorothiazide/spironolactone users showed altered serum sodium.
    • Furosemide users exhibited significant changes in uric acid, BUN, and creatinine.

    Conclusions:

    • Long-term diuretic use is high in ambulatory elderly individuals.
    • Diuretic therapy is associated with numerous, though often minor, alterations in biochemical values.
    • Careful monitoring of laboratory values is warranted in elderly patients on long-term diuretic therapy.
